Description

Essopine 40 mg Injection

Generic name: Esomeprazole
Strength: 40 mg
Dosage form: Lyophilized powder for IV injection/infusion
Drug class: Proton Pump Inhibitor (PPI)
Route: Intravenous (IV)

1. Indications

Esomeprazole IV is used mainly when oral treatment cannot be given, including:

  • GERD with erosive esophagitis
  • Short-term acid suppression when oral esomeprazole is not possible
  • Healing/management of acid-related esophageal disease
  • Prevention of rebleeding from gastric or duodenal ulcers following therapeutic endoscopy in adults.

2. Benefits

  • Strongly reduces gastric acid production.
  • Helps relieve acid reflux and heartburn.
  • Helps heal acid-related damage to the esophagus.
  • IV administration provides acid suppression when the patient cannot take medicine orally.
  • Can be used after endoscopic treatment of an acute bleeding peptic ulcer to reduce the risk of rebleeding.

3. Contraindications

Do not use in patients with:

  • Known hypersensitivity/allergy to esomeprazole
  • Allergy to substituted benzimidazoles or any formulation component
  • Certain interacting medicines, particularly nelfinavir; the exact interaction list depends on the product labeling.

4. Common Side Effects

Possible adverse effects include:

  • Headache
  • Diarrhea
  • Nausea
  • Abdominal pain
  • Constipation
  • Flatulence
  • Injection-site reactions

With prolonged PPI exposure, risks can include low magnesium, vitamin B12 deficiency, certain intestinal infections and fracture risk.

5. Warnings & Precautions

Use under medical supervision, particularly in patients with:

  • Severe liver impairment
  • Long-term PPI treatment
  • Previous osteoporosis/fracture risk
  • Unexplained weight loss or persistent gastrointestinal symptoms

Alarm symptoms such as difficulty swallowing, recurrent vomiting, vomiting blood, black stools, or unexplained weight loss require medical evaluation because acid suppression can sometimes mask symptoms of serious disease.

6. Usage / Dosage

For adults with GERD and erosive esophagitis when oral therapy is not possible:

20–40 mg IV once daily, generally for a short period, with conversion to oral therapy as soon as appropriate.

For the 40 mg dose, depending on the approved product instructions, the reconstituted medicine may be given:

  • IV injection: over at least 3 minutes
  • IV infusion: over approximately 10–30 minutes.

For prevention of Rebleeding after endoscopic treatment of an acute bleeding gastric/duodenal ulcer, a specialized regimen is used: 80 mg IV over 30 minutes followed by 8 mg/hour continuous infusion for 72 hours. This is a hospital-based regimen and is not the routine 40 mg dose.

Important: This product is for IV administration by a healthcare professional. The vial must be properly reconstituted and administered according to the manufacturer’s instructions. It should not be self-injected.
