Risperidone is a prescription medicine that affects dopamine and serotonin receptors in the brain. It is used for certain psychiatric and behavioral disorders.

Risperidone may be prescribed for:

• Schizophrenia
• Bipolar disorder, particularly manic or mixed episodes
• Certain severe behavioral/agitation symptoms associated with psychiatric disorders
• Irritability associated with autistic disorder in approved age groups

2. Benefits / Action

It works primarily by modulating dopamine D2 and serotonin 5-HT2A receptors.

3. Usage / Dosage

The dose depends greatly on the condition, age and individual response.

Adults with schizophrenia: treatment is generally started at a low dose and gradually adjusted. A commonly used target range is around 2–6 mg/day, divided or given once daily depending on the regimen.

Bipolar mania: doses are also individualized and titrated according to response.

Important: Riscalm 1 mg is a low-strength tablet commonly used for dose initiation or adjustment. The doctor should determine the exact dose and duration.

• May be taken with or without food.
• Take at the same time each day.
• Do not stop suddenly without medical advice.

4. Contraindications

Do not use risperidone in patients with:

• Known allergy/hypersensitivity to risperidone
• Hypersensitivity to formulation ingredients

Extra caution is required with cardiovascular disease, seizure disorders, Parkinson’s disease, diabetes, liver/kidney impairment and a history of stroke.

5. Side Effects

Common:

• Drowsiness or sedation
• Dizziness
• Headache
• Increased appetite and weight gain
• Constipation
• Dry mouth
• Fatigue
• Restlessness

Important adverse effects:

• Tremor or muscle stiffness
• Abnormal involuntary movements
• Increased prolactin
• Increased blood glucose/lipids
• Low blood pressure, particularly when standing

Seek urgent medical attention for: severe muscle rigidity with fever/confusion, seizures, severe allergic reaction, uncontrolled movements, or symptoms of stroke.

6. Warnings & Precautions

Risperidone should be used carefully in:

• Elderly patients with dementia-related psychosis — antipsychotics including risperidone are associated with increased mortality and are not approved for this use.
• Patients with cardiovascular disease or stroke risk
• Diabetes or metabolic syndrome
• Epilepsy
• Parkinson’s disease or Lewy body dementia
• Kidney or liver impairment

Regular monitoring of weight, blood pressure, glucose and lipids may be appropriate during treatment.

7. Drug Interactions

Important interactions can occur with:

• Other medicines that cause drowsiness
• Alcohol
• Blood-pressure-lowering medicines
• Certain antidepressants, including fluoxetine and paroxetine, which can increase risperidone concentrations
• Carbamazepine and other enzyme-inducing medicines, which can reduce risperidone concentrations

8. Pregnancy & Breastfeeding

Risperidone should be used during pregnancy or breastfeeding only when specifically advised by a healthcare professional. It can pass into breast milk.

9. Storage

Store in a cool, dry place, protected from excessive heat and moisture. Keep out of reach of children.
